The missing alarms <br /> include: <br /> -T3 (91) probe out alarm on 4/2/18. The monthly inspection report states "no alarms" <br /> -The L10 (UDC 7/8) sensor out alarm on 5/15/18 was documented by the designated operator as "due to liquid in 7 + <br /> 8". According to a work order on site, this alarm was caused by a contractor accidentally knocking over the position <br /> sensitive sensor. <br /> -The Q2 (87) gross line fail on 5/9/18 was documented by the designated operator as "due to liquid in 7 + 8 UDC". <br /> This would not be a possible cause for a gross line fail of a fuel (line. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Overfill prevention, tank tightness, and line tightness test information was left blank on the 10/19/18 inspection report. <br /> -Overfill prevention testing was marked as a 'NA'on the 12/14/18 inspection report and was not documented in Section <br /> III. <br /> -CERS ID was not included on the 10/19/18, 11/15/18, 12/14/18, or 1/9/19 inspection reports. <br /> -The 10/19/18 and 11/15/18 inspection reports left blank the question regarding UDC sensor locations. <br /> -Section VIII of the inspection reports marks 'yes'for leak detection alarms even in months where no alarms occurred. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on 1/23/19. This testing was required to be conducted by October 13, 2018. <br /> Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 22, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
