SECTION 76 <br />SPECIAL <br />Page 2 of 12 <br />(02/17) <br />2. As -built details of the well construction including elevation of existing grade, <br />Top of Casing, Enclosure Elevation with a notch and carriage bolt elevation <br />on the well skirt. All details shall be labeled and or have a legend. <br />1.03 PERMITS <br />A. Contractor shall obtain permits to drill and install ground water monitoring wells <br />from San Joaquin County Environmental Health Department (EHD). Contractor <br />shall call EHD Unit 6 and inquire about the permit procedure. <br />B. Contractor shall provide EHD all permit application documents. Contractor shall <br />provide EHD a work plan, showing the well location, well detail ,well installation <br />specifications and a health and safety plan for working at the site. <br />C. The Engineer will provide following information to the contractor to complete the <br />permit application for submittal to EHD. Facility SWIS #, Address, District No and <br />APN #. <br />D. Contractor shall pay for the permit fees and inspection fees to EHD. <br />E. The Contractor shall schedule inspection(s) of the well installation, with EHD Unit <br />6 supervisor or lead senior inspector at least 48 hours before starting any <br />construction. Call EHD main office at (209) 468-3420 to schedule inspection. <br />Note the name of the EHD inspector contacted. After scheduling inspection, the <br />Contractor shall provide the Engineer EHD's inspector's name and inspection <br />date/time. <br />F. Installation of ground water monitoring wells shall be approved by EHD inspector. <br />G. Installation of ground water monitoring wells without EHD inspection and prior <br />authorization will result in non-payment for that work. <br />H. Contractor shall notify underground service alert (USA) and have the work area <br />cleared by underground utility companies before drilling. Contractor will be held <br />liable for any damages caused by the Contractor by not following the guidelines <br />set by USA. The Contractor shall provide the USA ticket numbers to the <br />Engineer 24 hours before drilling. The County will also notify USA. <br />1.04 INSPECTION <br />A. For the purpose of this section, installation refers to placing material in a <br />borehole. Well drilling does not require EHD inspection. <br />LANDFILL AREA 513 -SEC 76 -GW Well Specs <br />
