5208 S.Lawton <br /> 'MA <br /> Tulsa OK,74107 <br /> 918-998-0862 Date: 12/15/2016 <br /> AC U R E N Location: Owens Brockway Tray CA <br /> Equip.#: Emergency Generator C <br /> Cert.Level: API-653#36530 <br /> Inspector: Gregory Fann <br /> Examination Report <br /> Scope: <br /> 1)External visual inspection. <br /> 2)Ultrasonic thickness examination of shell,roof and floor. <br /> Summary: <br /> The subject 195 gallon capacity atmospheric storage tank was inspected per the above scope and evaluated per the specified STI SP-001. <br /> Inspection of the tank bottom was not possible due to proximity of the floor.UT examination was limited due to surface roughness from corrosion.Further <br /> details may be referenced in the attached Checklist,Calculation sheet,TML report,Image sheet,or Drawing. <br /> Results: <br /> 1)Oil,debris and feces contamination. <br /> 2)Insufficient containment. <br /> 3)Non functioning,illegible level gauge. <br /> 4)Moderate local corrosion. <br /> 5)Light general corrosion. <br /> Recommendations: <br /> 1)Remove contaminant from tank,repair leaking component.(do not leave absorbs on tank,they will accelerate corrosion) <br /> 2)Add adequate containment around tank to reduce consequence of failure. <br /> 3)Replace level gauge. <br /> 4)Remove scale and re-coat to prevent further corrosion. <br /> 5)Remove scale and re-coat to prevent further corrosion. <br /> 6)Continue scheduled inspection program per STI SP-001 and O-I company policies and procedures. <br />
