Mark Koehing, Lab Manager, arrived at the sterilizer. <br />Once the bags were pushed into the compactor, the compactor was <br />opened. The bags were not sterilized. One strip had turned <br />slightly black, however, the other strips were still white. <br />Again, blood could be observed through the clear bag and all bags <br />were moist. <br />Mr. Wilson stated, "We have a problem here. The problem is with <br />this machine and your procedures. It is easy to go from one <br />cycle to the compactor to waste bin without anyone ever checking <br />the bags to verify sterilization." <br />Mr. Wilson said, "What you need to do is get a medical waste <br />hauler in to pick up the waste over the weekend until the <br />San -i -Pak can be repaired. Then, you need to get the San -i -Pak <br />repaired. I recommend you get a certification from the repair <br />man. Finally, test the center contents of one bag in the <br />laboratory to see if it is sterile." <br />Ms. Knowles accompanied Mr. Wilson and myself out to Magnolia <br />Street. She said, "Let me go over what I need to do one more <br />time. Get a medical waste hauler, get the machine repaired, get <br />a certificate from the repair man, and run laboratory tests on <br />the center of one bag before going into operation." <br />Mr. Wilson, "That sounds right." <br />At 12:50 p.m., Mr. Wilson and I departed Dameron Hospital. <br />REPORT OF VIOLATIONS <br />1. California Health and Safety Code, Chapter 6.1, Article 8, <br />Section 25088. Demeron Hospital has used a compactor on <br />medical waste before it is determined that the waste had <br />been properly treated and rendered solid waste. <br />2. California Health and Safety Code, Chapter 6.1, Article 9, <br />Section 25090 (A) (c) (1); (B) c (2); (C) c (3); and (d) <br />(c) (4). Demeron Hospital has standard written operating <br />procedures, however, Dameron has no procedures to monitor <br />San-i-Pac sterilizer performance to indicate adequate <br />sterilization. Employees do not know proper time, <br />temperature, or pressure for medical waste treatment. <br />Dameron Hospital has not checked indicating thermometers <br />during the cycle. <br />Dameron Hospital is not using heat sensitive tape on bags <br />to indicate adequate sterilization conditions. <br />4 <br />
