Complaint Investigation Summary Report#:5105
COMPLAINT ID: COWPTSQMU Site Location: 10749 W WOODBRIDGE RD
Activity Summary
Activity Date Recorded by
and Wes(Wilbur Ellis). Observed the failed poly tank located on the east side of a
warehouse/shop building, the spill impacting a large area of compact ground soil,
on-site equipment, possibly a few olive trees on the edge of the orchard, and
possibly the drainage pipe and ditch just south of the failed tank and the drainage
pipe and ditch west of the spill. Left site for EHD.
Arrived back at EHD. Emailed Isabella the Safety Data Sheet for the spilled
material. Updated Elena Manzo, Ld Sr REHS. Emailed Robert McClellon, Solid
Waste and Small Public Water Systems Unit Program Coordinator, and Natalia
Subbotniova, Ld Sr REHS, a copy of the Cal OES spill report and update and
notified them that the spill may or may not have impacted drainage ditches that
lead to Sycamore Slough and the Mokelumne River.
03/29/2021 Received a call from Chris Feathers (Patriot)who confirmed the clean-up will not FLORIDO
begin until the property owner approves Patriot's scope of work, which is to be
completed and sent to the property owner today. Chris summarized the general
scope of work, stating Patriot would place the excavated soil into roll-off bins, and
take a composite sample (4 samples from one bin combined into one sample) of
each bin for waste analysis and profiling. Chris also stated he would inform Jamie
Dobbs to perform sampling of the drainage ditches to prove whether or not the
spill impacted the ditches.
Received a call from Derek Smith (Patriot) informing me that he is having trouble
finding a local lab that will test for the urea ammonium. After further discussion,
Derek stated he will try to find a lab that can test for the exact spill material,
UAN-32 Urea Ammonium Nitrate.
Derek called me back stating he found a lab, ExelChem Laboratories in Rocklin
that can conduct the ditch soil sample analysis for the spill material. I confirmed
that the lab is ELAP certified.
Called Trevor(property representative) and asked him if the drainage ditches on
the property lead to any nearby waterways. Trevor stated the ditches on his
property lead to a District ditch, a pump, and then to the Mokelumne River. Trevor
did not know the name of the District that owns the ditch.
Generated Prop 65 and Emergency Response Record form.
Phone call discussion with Kristen (Patriot) and Isabella (Water Board) regarding
the final site assessment done today by Jamie Dobb's (Patriot). Kristen confirmed
Jamie took soil and water samples today of the two possibly impacted drainage
ditches to determine if the ditches were impacted by the spill. Kristen also stated
once the property owner approves the clean-up scope of work, it may take a few
days before the clean up can begin due to scheduling of the necessary equipment
and personnel. Kristen to email me and Isabella a copy of Patriot's scope of work.
5105.rpt Page 5 of 7
