Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COWPTSQMU Site Location: 10749 W WOODBRIDGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> and Wes(Wilbur Ellis). Observed the failed poly tank located on the east side of a <br /> warehouse/shop building, the spill impacting a large area of compact ground soil, <br /> on-site equipment, possibly a few olive trees on the edge of the orchard, and <br /> possibly the drainage pipe and ditch just south of the failed tank and the drainage <br /> pipe and ditch west of the spill. Left site for EHD. <br /> Arrived back at EHD. Emailed Isabella the Safety Data Sheet for the spilled <br /> material. Updated Elena Manzo, Ld Sr REHS. Emailed Robert McClellon, Solid <br /> Waste and Small Public Water Systems Unit Program Coordinator, and Natalia <br /> Subbotniova, Ld Sr REHS, a copy of the Cal OES spill report and update and <br /> notified them that the spill may or may not have impacted drainage ditches that <br /> lead to Sycamore Slough and the Mokelumne River. <br /> 03/29/2021 Received a call from Chris Feathers (Patriot)who confirmed the clean-up will not FLORIDO <br /> begin until the property owner approves Patriot's scope of work, which is to be <br /> completed and sent to the property owner today. Chris summarized the general <br /> scope of work, stating Patriot would place the excavated soil into roll-off bins, and <br /> take a composite sample (4 samples from one bin combined into one sample) of <br /> each bin for waste analysis and profiling. Chris also stated he would inform Jamie <br /> Dobbs to perform sampling of the drainage ditches to prove whether or not the <br /> spill impacted the ditches. <br /> Received a call from Derek Smith (Patriot) informing me that he is having trouble <br /> finding a local lab that will test for the urea ammonium. After further discussion, <br /> Derek stated he will try to find a lab that can test for the exact spill material, <br /> UAN-32 Urea Ammonium Nitrate. <br /> Derek called me back stating he found a lab, ExelChem Laboratories in Rocklin <br /> that can conduct the ditch soil sample analysis for the spill material. I confirmed <br /> that the lab is ELAP certified. <br /> Called Trevor(property representative) and asked him if the drainage ditches on <br /> the property lead to any nearby waterways. Trevor stated the ditches on his <br /> property lead to a District ditch, a pump, and then to the Mokelumne River. Trevor <br /> did not know the name of the District that owns the ditch. <br /> Generated Prop 65 and Emergency Response Record form. <br /> Phone call discussion with Kristen (Patriot) and Isabella (Water Board) regarding <br /> the final site assessment done today by Jamie Dobb's (Patriot). Kristen confirmed <br /> Jamie took soil and water samples today of the two possibly impacted drainage <br /> ditches to determine if the ditches were impacted by the spill. Kristen also stated <br /> once the property owner approves the clean-up scope of work, it may take a few <br /> days before the clean up can begin due to scheduling of the necessary equipment <br /> and personnel.
