Whiskey Slough Road <br /> Holt, CA 95234 <br /> <br />Dear Mr. Ramsey, <br />On May 29, 2018 representatives of the San Joaquin County Environmental Health <br />Department (“EHD”), which is the certified unified program agency (“CUPA”) in this <br />jurisdiction, conducted a Hazardous Materials Program inspection at 3401 S. Whiskey Slough <br />Road, Holt CA 95234 (hereinafter "Facility"). During the inspection, EHD discovered violations <br />of law and, based on those findings, EHD is prepared to bring an administrative action against <br />you pursuant to California Health and Safety Code Section 25404.1.1. The administrative <br />action is to ensure compliance with applicable statutes and assess penalties for violations. A <br />copy of the inspection report is enclosed. <br />EHD has found that the Facility is/was in violation of the following regulations: <br />1. HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or <br />Owner/Operator Identification page <br />2. HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory <br />for all reportable materials on site <br />3. HSC 25505(a)(2) Failed to complete and/or submit a site map with all required <br />content <br />4. HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures <br />for a release or a threatened release <br />5. HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or <br />adequate for facility <br />6. HSC 25508(a)(1) Failed to annually review and certify that the business plan is <br />complete, accurate, and up to date <br />7. HSC 25508.1 (a)-(f) Failed to submit revisions to business plan within 30 days of
