Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: C00052900 Site Location: 16777 HOWLAND RD <br />Activity Date <br />Activity Summary <br />Recorded by <br />11/03/2020 Called Ryan Mock (Simplot) twice for status updates: MURO <br />1st call 10:53 AM: <br />- Simplot has been monitoring the tank hourly. <br />- The leak doesn't seem to be increasing hourly <br />- He is unable to determine how much has released as the tank's electronic level <br />is difficult to track small leaks <br />- Tank has bladder type system <br />2nd call 4:45 PM: <br />- He doesn't have an amount of product released. The release doesn't seem to be <br />increasing. <br />- No off-loading taking place yet, they are still working on lining up trucks and/or <br />railcars for off loading <br />- They are not providing status updates to the fire department because the fire <br />department didn't request updates. <br />Received SDS for the Defluorinated Phosphoric Acid from Elianna Florido (EHD). <br />11/04/2020 Submitted paperwork for overtime worked on 11/2/20. FLORIDO <br />11/04/2020 Updated Elena Manzo (EHD), Melissa Nissim (EHD), and Elianna Florido (EHD) <br />of current complaint status. <br />MURO <br />11/04/2020 Generated Prop 65 and Emergency Response Record. MURO <br />Received the following update from Ryan Mock (Simplot): <br />- There doesn't appear to be any more of a release amount than two days ago. <br />- They began offloading 22 tons of product onto a truck late yesterday, 11/3. <br />- They started to offload onto another tank today. <br />- Facility has a railcar onsite which has capacity of 99 tons. <br />- Facility plans to obtain two more trucks <br />- They project to be finished off-loading tomorrow mid-morning. <br />11/05/2020 Received a Follow Up Notice as an attachment from Ryan Mock (Simplot). The <br />attachment is an 'Emergency Release Follow-Up Notice Reporting Form' which <br />contains details of the release, actions taken, and is signed at the end as a <br />certification. <br />MURO <br />Elena Manzo (EHD) and Brendan Kenny (Central Valley Regional Water Quality <br />Control Board) Brendan.Kenny@waterboards.ca.gov were both copied on the <br />email. <br />11/05/2020 Updated Elena Manzo on current complaint status and discussed.
