Hwy 99, Stockton - UST Install <br />Good Afternoon Mr. Velasco, <br />I'm not sure if you received my email. I understand that Mr. Sandoval was in our office last week <br />checking on the status of this installation. I have Stacy's list below of items that are still missing on the <br />application. I found some of the forms in our office like the application and ELD testing but the below <br />are still missing. <br />1) Page 3 of the Application for the Underground Storage Tank Installation Permit: <br />Owner/operator information is outdated. <br />2) Complete manufacturer certifications were not submitted. Provide current contractor's training <br />certifications for installation and testing of all proposed UST equipment/components, including <br />tanks & sumps, flex connectors, piping, spill buckets, sump shields & lids, leak detectors, and <br />overfill prevention. A number of the attached certifications have expired and there is not a <br />certification for each part being installed. <br />3) Provide manufacturer's cut sheets for all proposed UST system components, including piping, <br />flex connectors, STP & fill sumps, UDCs, vent sumps, penetrations, spill buckets, and overfill <br />prevention. 1 found a link in one of the emails but it says that it is an "invalid link". <br />4) Provide tank charts for the tanks being used. Sheets M5.3.04, M5.3.05, M5.3.06., & M5.6.03 are <br />different from the tank charts needed. Tank carts are a large table of values that list the dipstick <br />value associated with the gallons in the tank. /found tank charts in the papers for this install but <br />23 <br />
