11203 We have located the records for 1/16/19 overfill prevention equipment inspection results (91 <br />flapper) and work order for the 3/22/21 sensor replacement in UDC 7/8 and are sending a copy to EHD, <br />the work order number is 99130. <br />B208 We will make sure our designated operator perforins the inspections within the required time <br />frames. We will also make sure our designated operator includes all the required information on the <br />reports for the visual inspections. We will ensure they will also be recorded on the most current form <br />and that all the required documents are attached to future designated operator inspection reports. <br />11209 We have updated the Facility Employee Training Certificate with 2 separate forms. One for Aug. <br />2020 and one for Aug. 2021, we have added the accurate information and submitted a copy of the <br />updated forms to the EHD. <br />#214 We have obtained a permit from the EHD on 11/11/21. <br />#308 We will make sure our monitoring equipment will be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak. <br />#309 We will make sure our monitoring equipment will be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak.
