The report shall include species encountered, capture methods; <br />methods used for handling, stress minimization, equipment cleaning and <br />disinfection; sizes of holding facilities; descriptions of relocation sites; and all <br />instances of mortality and injury. <br />4.2 Monthly Monitoring and Compliance Reports. The Qualified Biologist shall submit a <br />monthly report electronically to CDFW that includes the following items: <br />• Notification number, <br />• Begin and end times of daily worksite monitoring, <br />• Begin and end times of all activities associated with the project, <br />• Survey results, <br />• A summarized description of whether compliance for all avoidance and <br />minimization measures has been met, <br />• Recommendations to achieve compliance of any avoidance and minimization <br />measures that have not been met, <br />• Fish and wildlife species (and their sign) observed during monitoring, <br />• Any instances of capture and relocation of fish and wildlife, <br />• Any avoidance behaviors of fish and wildlife, <br />• Photo documentation of site conditions. <br />4.3 Final Construction Report. A final construction report shall be submitted to CDFW <br />within 30 calendar days of the date of completion of all construction activity. This <br />report shall detail dates construction occurred, pertinent information concerning the <br />success of the project in meeting avoidance and minimization measures, mitigation <br />measures, conservation measures, detailed summaries of any deceased wildlife <br />discovered at the site, and/or any detailed summaries required in the measures of <br />this Agreement. An explanation of failure to meet such measures as specified in <br />this Agreement shall also be included, if applicable. <br />4.4 Annual Riparian/Tidal Wetland Remediation Monitoring Report. An annual <br />monitoring report shall be submitted to CDFW by December 31 of each monitoring <br />year. The report shall include an assessment of the remediation monitoring, <br />overall vegetation cover, , invasive species establishment (if any), invasive species <br />cover by percent, establishment of riparian vegetation and tidal wetland vegetation, <br />and photo documentation. <br />4.5 Notification to the California Natural Diversity Database (CNDDB). If any listed, <br />rare, or special status species are detected during project surveys or on or around <br />the project site during project activities, Permittee shall submit CNDDB Field <br />Survey Forms to CDFW in the manner described at the CNDDB website <br />(http://www.dfg.ca.gov/biogeodata/cnddbn within 30 days of the sightings. Copies <br />of such submittals shall also be submitted to the CDFW regional office as specified <br />below. <br />
