After a review of the proposals the following <br />items must be incorporated. <br />1. Application for a drilling permit must be submitted to the <br />San Joaquin Local Health District with a detailed work schedule <br />and map showing boring holes, depths, and location. A valid <br />permit must be obtained prior to commencing any drilling <br />operation. <br />2. The annular seal for the monitoring wells are to be filled from <br />the bottom using a pressure grout technique. Gravity backfill <br />of the grout seal is not an accepted technique for the annular <br />seal depths proposed for the monitoring well at this site. (See <br />enclosed guidelines dated 7-28-87). <br />3.
