Map Key Number of Direction Distance Elev/Diff Site DS <br />Records (mi/ft) (ft) <br />On 26JUN07, Shea Homes and DTSC executed an amendment to the existing VCA to address expansion of the site area to include a specified portion <br />of the adjacent property identified by APN 209-450-02 as the future (at the time) Great Valley Parkway. <br />On 9JUL07, Condor produced a Removal Action Workplan (RAW) which identified the selected method for remediation of the surface soil containing <br />residual Dieldrin concentrations from historic practices at the site. The selected remedy was surface soil excavation of approximately 40.6 acres to a <br />depth of 0.5ft bgs using paddle -wheel type scrapers. The total in-place volume of soil proposed for removal was approximately 33,000 in-place cubic <br />yards (cys). Excavated soil would be encapsulated beneath the Great Valley Parkway in areas without planned subsurface utilities. After excavation, <br />confirmation and waste characterization samples would be collected. <br />On 10JUL07, DTSC's Cleanup Operations Division requested that the Human and Ecological Risk Division (HERD), Industrial Hygiene and Safety <br />Branch (IHSB) review the soil/site management plan for the RAW for the site. Specifically, HERD/IHSB reviewed sections 6.3 (SJVAPCD requirements) <br />and 7.4 (Air and Meteorological Monitoring Plan). On 11 JUL07, HERD/IHSB issued a memorandum with its conclusions and recommendations. <br />On 18JUL07, DTSC approved the Phase II Environmental Assessment Report for the Southern portion of Neighborhood C (APNs 209-450-03 and <br />portion of 209-450-02). In accordance with the VCA (Docket No. HAS -VCA 06/07-147), DTSC accepted submittal of the Phase II Report as equivalent <br />documentation to a Preliminary Endangerment Assessment (PEA). <br />On 23AUG07, Shea Homes requested implementation of the Removal Action Work Plan be postponed indefinitely due to the economic slowdown in the <br />residential real estate market. The Site property was fenced and did not pose a threat to public health or the environment. DTSC approved the <br />postponement on the condition that a public notice was filed in the local area newspaper, that Shea Homes mail out a copy of the public notice to all <br />parties on the DTSC approved site mailing list, and that a written notice be provided to DTSC (at least 30 days) and to all parties on the DTSC approved <br />site mailing list (at least 5 days) prior to the commencement of the RAW implementation action. <br />On 24SEP07, DTSC signed a California Environmental Quality Act (CEQA) Notice of Exemption (NOE) which was filed with the Office of Planning and <br />Research (OPR), State Clearinghouse on 27SEP07. The CEQA NOE underwent a 30 -day public comment period from 23JUL07 to 22AUG07 which <br />allowed the general public an opportunity to review the RAW and NOE, and to contact DTSC if they had questions or comments regarding the proposed <br />project. One public inquiry was received, the result of which, did not require that any changes be made to the RAW. The RAW and NOE were then <br />considered final and approved. <br />On 11 OCT07, DTSC approved the RAW (conditionally) pending submittal of two copies of the final RAW to DTSC and one copy to the information <br />repository. <br />On 13SEP13, the Proponent notified DTSC that they wanted to reactivate the project and enter into a VCA Amendment with DTSC to allow for <br />completion of the Site cleanup. DTSC produced an amendment to the VCA and mailed out the official amendment for the Proponent's signature. The <br />proponent signed and returned the document for DTSC's counter signature and execution. <br />On 11 OCT13, DTSC mailed out a fully executed copy of the amended VCA to the Proponent. DTSC's letter noted the commencement date for the <br />fieldwork to be 140CT13. <br />By 23DEC13, the Dieldrin contaminated soil was encapsulated and the Proponent was preparing for the installation of the road at the site. <br />On 8AUG16, a Removal Action Completion Report (RACR) was produced for the site by Condor. The RACR concluded that the removal action at the <br />site was conducted in the fall of 2013 and was conducted in general accordance with the RAW dated 9JUL07. The RACR also noted that the final <br />installation of the roadway was completed in August 2016 which completed the encasement area cover for the project. Based on the analysis of samples <br />collected for the confirmation and waste characterization, Condor recommended that the residential portion of the site be approved for occupancy and <br />that a Land Use Covenant be executed for the waste soil encapsulation area. <br />On 8MAR17, DTSC approved an Operations and Maintenance Plan (OMP) produced by Condor (March 2017) for the site. The OMP outlined the <br />procedure for the long term maintenance of the site and included the Soil Management Plan (SMP) therein. <br />On 28JULl7, an Irrevocable Letter of Credit was produced by US Bank and provided to DTSC for the amount of the FA as estimated by DTSC's <br />Engineering and Special Projects Office (ESPO) for the Operations and Maintenance activities necessary to maintain the remedy selected for the site. <br />On 23AUG17, DTSC mailed out a signed copy of the Land Use Covenant (LUC) for the site outlining the land use restrictions for the site. The LUC was <br />recorded by the County of San Joaquin on 1 SEPI 7 at 13:59:32 hours. <br />On 31AUG17, DTSC mailed out a fully executed Operations and Maintenance Agreement (OMA) for the site to the Proponent. The OMA was signed <br />and considered fully executed on 29AUG17. In this mailing, DTSC also took the opportunity to note the Financial Assurance (FA) (OMA: Section 9) for <br />the site as, "verified and fully in effect" at this time. <br />On 13SEP17, DTSC produced a Certification of Remedial Action for the site and mailed out a certification letter to the Proponent which certifies that the <br />remediation action at the site is complete. <br />In late September 2017, the site was transferred over to a local government agency - the Mountain House Community Services District (MHCSD). The <br />proponent asked that a new OMA/OMP needed to be produced for MHCSD and that they would be taking over as Proponent for the site. A new <br />OMA/OMP was produced with the assistance of DTSC's Office of Legal Counsel (OLC) and sent to MHCSD for signature. There was no further action <br />on this change until February 2018. <br />On 22AUG18. DTSC supplied MHCSD with the final revised OMA. <br />On 6SEP18, DTSC received the final signed OMA from MHCSD and countersigned the OMA and supplied a copy to MHCSD. <br />erisinfo.com I Environmental Risk Information Services Order No: 21080200152 <br />
