Complaint Investigation Summary <br /> Report #. 5105 <br />COMPLAINT ID: C00051856 Site Location: 9015 W WALNUT GROVE RD <br />Activity Summary <br />Activity Date Recorded by <br />04/16/2020 Continued from morning after hours. MURO <br />8:00 AM En route to 9015 W Walnut Rd. <br />8:26 AM Arrived at spill area. Site is adjacent to Chevron gas station. Cleanup was <br />conducted by Dave Larsen (PG&E). The non-PCB mineral oil on the transformer <br />pad was cleaned using with absorbent and penetone. One 55-gallon drum was <br />generated and taken to 4044 West Lane, Stockton PG&E storage area. For the <br />mineral oil that landed on the soil, Dave determined a USA dig notification was <br />needed for excavating the impacted soil. Dave coned off the spill area pending <br />USA dig clearance for removal of impacted soil. Excavation is tentatively <br />scheduled for Wednesday, April 22, 2020. <br />04/17/2020 Completed Prop 65 and Emergency Response Record. MURO <br />04/22/2020 8:33 AM Called Alex Steele (PG&E), 209-337-8902 for status on excavation <br />tentatively scheduled for today. He will call Dave Larsen (PG&E) then call me back <br />with update. <br />8:45 AM Alex Steele called to report that Dave Larsen has arrived and is setting <br />up. He will not know what time the excavation will be complete. It may be later this <br />afternoon or if not, they will have to return tomorrow. <br />Alex will contact us to update us once estimated completion is known. <br />1:00 pm Alex Steele called. He asked to meet Dave Larsen at the excavation site <br />at 3:30 pm - The excavation will be complete by then. <br />3:06 pm en route to meet with Dave for verification of cleanup via excavation. <br />3:30 pm Arrived. Dave Larsen excavated the area around the pad-mounted <br />transformer using a Kubota tractor. <br />Dave Larsen indicated that seven 55-gallon drums of mineral oil contaminated soil <br />were generated from the excavation. The drums were labeled as hazardous <br />waste. They will be taken to 4040 West Lane, Stockton. <br />The dimensions of the excavation are as follows: <br />North side of pad-mounted transformer: 6 x 8 feet, 2 feet deep. <br />South side of of pad-mounted transformer: 10 x 2 feet, 2 feet deep. <br />Front side of pad-mounted transformer: 10 x 2 feet, 1 foot deep. <br />MURO <br />Cleanup of the mineral oil release by excavation was adequate. <br />04/23/2020 Generated Photo log MURO <br />04/24/2020 Completed the narrative for 4/22/20. MURO <br />05/14/2020 Generated photo log of post cleanup. MURO <br />Emailed Alex Steele requesting analytical lab report. <br />05/26/2020 Reviewed Castle Analytical Laboratoryl lab results received on 5-26-20 from Alex MURO <br />Steele, (PGE). <br />The report indicates that the sample taken from 9015 W Walnut Grove on 4-18-20 <br />tested as non detect for PCBS. <br />5105 rpt <br /> Page 2 of 3
