San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CVS PHARMACY#3908 1885 W ELEVENTH St, TRACY September 25, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16(a-c) Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> The following was found in the store's hazardous waste(HW)storage area: <br /> -A Tell Me Your Wish Lucky Ariul hand cream was found in a StrongPak bag inside a Flammables HW tote with <br /> accumulation start date 9/24/18(IMG 1993-1998).The item scanned as"Dispose in Dumpster."According to Mary <br /> Young,this item was placed in a HW tote because it was believed that the item should be handled as HW rather <br /> than disposed of in the dumpster.When asked who employees call when questions arise regarding the waste <br /> categorization of items, Mary did not know to call the CVS Safety Hotline or the CVS Environmental Contact. <br /> -A Toxic HW tote with accumulation start date 8/28/18 had a HW label indicating only solid OTC Pharmaceuticals <br /> were contained in the tote(IMG 2002). However,the following items were found: <br /> -Herbal Essences Rose Hips Shampoo, scanned as Toxic HW(IMG 2003-2005). <br /> -Not Your Mother's Beach Babe Texturing Sea Salt Spray,scanned as Toxic HW(IMG 2006-2007). <br /> -Two bottles of Tresemme Heat Tamer Leave In Spray, scanned as Toxic HW(IMG 2008-2009). One of these <br /> bottles was leaking inside the single StrongPak bag,which did not contain absorbent. Mary did not why the bottle <br /> was placed into only one StrongPak bag without absorbent. Mary double-bagged the leaking item at the time of <br /> inspection(IMG 2031). Mary was unable to put enough absorbent into the StrongPak bag to absorb the leakage, <br /> due to a lack of absorbent in the Spill Kit(IMG 2010-2015). Mary stated she needs to order more absorbent for the <br /> spill kit. <br /> -Schick Hydro Sense Shave Cream,scanned as Toxic HW(IMG 2016-2017). <br /> -MD Complete Anti-Aging Restoring Cleanser&Makeup Remover, scanned as Toxic HW(IMG 2018-2019). <br /> -Shea Moisture 100%Virgin Coconut Oil Bath Sugar Cubes, scanned as Dispose in Dumpster(IMG 2023-2025). <br /> -Dove Cream Oil body lotion, scanned as Toxic HW(IMG 2026-2027). <br /> -Three StrongPak bag's contained what appeared to be leaking make-up items(IMG 2028-2030).The bags did <br /> not contain absorbent, and the items were single-bagged. Mary did not know why the items were placed into only <br /> one StrongPak bag without absorbent. Mary double-bagged each of the leaking items at the time of inspection(IMG <br /> 2030). Mary did not place any absorbent into the StrongPak bags to absorb the leakage,due to a lack of absorbent <br /> in the Spill Kit(IMG 2010-2015). Mary stated she needs to order more absorbent for the spill kit. <br /> -Orange Glo 4-in-1 Monthly Polish Hardwood Floor Polish, scanned as Toxic HW(IMG 2033-2034). <br /> -Persil ProClean liquid laundry detergent, scanned as Toxic HW, and was found leaking in a single StrongPak bag <br /> without absorbent(IMG 2034, 2036). Mary did not know why this item was placed into only one StrongPak bag <br /> without absorbent. Mary double-bagged the item at the time of inspection(IMG 2035). Mary did not place any <br /> absorbent into the StrongPak bag to absorb the leakage, due to a lack of absorbent in the Spill Kit(IMG 2010-2015). <br /> Mary stated she needs to order more absorbent for the spill kit. <br /> -Hibiclens Antiseptic Skin Cleanser, scanned as Toxic HW(IMG 2037-2038). <br /> -Eva NYC Mane Magic 10-in-1 Primer, scanned as Flammable HW(IMG 2039-2040). Mary placed this item into a <br /> Flammable HW tote at the time of inspection. <br /> -A Graduate Musical Ball, scanned as Toxic HW(IMG 2041-2044), although the item contained batteries.When <br /> asked why this item was placed in the Toxic HW tote, rather than a Universal Waste tote, Mary stated because the <br /> employee likely just followed what the scanner prompted them to do, rather than question if the item should go into <br /> a Universal HW tote. <br /> -Four Miss Spa Bee Venom Facial Sheet Masks, scanned as Toxic HW(IMG 2046-2047). <br /> All of the items listed above were identified as waste by Mary at the time of inspection.
