San Joaquin County Environmental Health Department 1868 East Hazelton Avenue, Stockton, California 95205-6232 Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd RCRA Large Quantity Hazardous Waste Generator Inspection Report Facility Name: Facility Address: Date: CVS PHARMACY#3908 1885 W ELEVENTH St,TRACY September 25, 2018 SUMMARY OF VIOLATIONS (CLASS I,CLASS II,or MINOR-Notice to Comply) Item# Remarks regarding the waste categorization of items, Mary did not know to call the CVS Safety Hotline or the CVS Environmental Contact. -A Bubble Mate Foot Spa was found open with a Toxic HW label, however the item, including the item's box,was not contained in a HW tote(IMG 2049-2050).The boxed item did not appear to fit into the totes CVS uses for its HW. -A plastic Teenage Mutant Ninja Turtles cup,scanned as Universal Waste(UW),was found in a StrongPak bag inside a Universal Waste tote with accumulation start date 9/17/18(IMG 2052-2056).The UW label on the tote indicated that electronic devices were contained in the tote. Mary was not sure why the cup was categorized as UW. When asked who employees call when questions arise regarding the waste categorization of items, Mary did not know to call the CVS Safety Hotline or the CVS Environmental Contact. All of the items listed above were identified as waste by Mary Young at the time of inspection. The following observations were made in the pharmacy: -The pharmacy's HW tote contained loose pills,empty Warfarin containers, and used flu vaccine vials, but did not have the contents marked on the HW label(IMG 2059). Trixia Thompson, a pharmacy technician, marked the contents on the HW label at the time of inspection(IMG 2061). -An open blue bin labeled with"Weekly StrongPak Accumulation Bin,""Capped Amber Vials(bag liquids),"and "Loose Pills(bagged)"was found containing bags of loose pills and vials with pills(IMG 2060,2062-2063). According to Trixia, loose pills and other pharmaceuticals that are to be managed as HW are first placed in this blue bin until processed and put into the pharmacy HW tote every Friday, at the latest. Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual review of the initial training received. At a minimum, the training program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where applicable: 1. procedures for using, inspecting, repairing,and replacing facility emergency and monitoring equipment; 2. key parameters for automatic waste feed cut-off systems; 3. communications or alarm systems; 4. response to fires or explosions; 5. response to ground-water contamination incidents; and 6. shutdown of operations. The owner or operator shall maintain the following documents and records at the facility: 1. the job title for each position at the facility related to hazardous waste management, and the name of each employee filling each job; 2. written job description for each position listed above; 3. written description of the type and amount of both introductory and continuing training that will be given to each person filing positions listed above; 4. records that document that the training has been given to and completed by facility personnel. Training records on current employees shall be kept until closure of the facility. Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial or refresher hazardous waste training. Submit a copy of the roster and the syllabus to the EHD.
