Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COOMGBEJX Site Location: 3003 NAVY DR <br />Activity Summary <br />Activity Date <br /> <br />Recorded by <br /> <br />07/02/2021 Received email from Ruthanne Walker (Marathon Petroleum) with copy of <br />manifest and photos of hazardous waste labels which were emailed to her by <br />Derek Smith (Patriot Environmental). I requested copy of the final (signed) <br />manifest once it is received. <br />07/02/2021 Received request via email from Ruthanne Walker that if any fines are associated <br />with EHD's response, that we issue to the tanker truck drivers as they were both at <br />fault. I replied Paul Bruss indicated that the tanker truck drivers were at fault, <br />however, he asked that I place Marathon Petroleum as the responsible party. I <br />also let her know that amending the responsible party (RP) information on the <br />complaint, notice to abate, emergency response record, etc., will incur additional <br />billable time for the RPs. I would need: - Business name <br />- Phone Number <br />Mailing Address <br />Contact person's name, email address, and phone number <br />I will need to discuss with the managers before making any changes to the <br />complaint. <br />MURO <br />MURO <br />07/13/2021 Discussed Marathon Petroleum's request to change responsible party information MURO <br />with Elena Manzo. <br />Emailed Ruthanne Walker requesting responsible party information. Ruthanne <br />replied and stated that she has not discussed with Marathon. She wishes to keep <br />as is if the notice to abate is basic procedure for all facilities in the county. <br />Discussed with Elena Manzo. Emailed response to Ruthanne. Let her know while <br />no penalties remain, there is billable time associated with EHD's response to the <br />incident. <br />07/16/2021 Emailed Ruthanne Walker (Marathon Petroleum). Requested final disposal MURO <br />record. <br />07/23/2021 Reviewed complaint, emailed complaint narrative summary to Bencomo, MANZO <br />Isabella@Waterboards <Isabella.Bencomo@Waterboards.ca.gov> (per her <br />request), cc to M. Nissim (EHD CUPA program coordinator). <br />09/16/2021 Reviewed complaint narrative for typos or discrepancies.
