Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COWZIRJVO Site Location: 14749 N THORNTON RD <br />Activity Summary <br />Activity Date Recorded by <br />12/08/2020 11:08 AM Received notification from Ben Khatirine (3 B's Truck and Auto Plaza MURO <br />owner's son) of approximately 40 — 50 gallons renewable diesel spill via phone call <br />to EHD. <br />I called Ben Khatirine, 916-261-6194. <br />Ben reported: <br />- A truck crashed into fuel dispenser # 6 causing the diesel release onto pavement <br />and storm drain. <br />- The storm drain liquid goes to the onsite clarifier. No drinking water is impacted. <br />The clarifier is controlled by a switch control to empty. The liquid contained in the <br />clarifier, when switch is powered for emptying, goes to a drainage ditch (west of <br />the parcel). <br />- Responsible party is Sundance Transportation Inc. IP <br />12:40 PM Arrived to site of release, met with Ben Khatirine and Francisco Avila (3 <br />B's Tuck and Auto Plaza manager). Observed the impacted fuel dispenser with the <br />containment under open. Observed damaged overhead diesel exhaust fluid (DEF) <br />additive pipe with release on the pavement. Diesel spill was cleaned up by <br />Francisco using absorbent immediately after emergency shut-off was initiated by <br />Francisco. Francisco reported the diesel from the under dispenser containment <br />(UDC) was removed and put it inside a 55-gallon drum. Observed pavement near <br />UDC #'s 4, 5, and 6 had diesel exhaust fluid that has not yet cleaned up. Observed <br />some diesel came out of the valve beneath the fuel dispenser. Francisco stated <br />that air pockets in the line cause a little release of diesel, and he will clean it up. <br />Francisco escorted me to the power shut-offs. Verified the switch to dispenser #6 <br />was shutoff and locked with padlock. Verified the power to the clarifier to be shut <br />off and locked with padlock. Verified the overhead DEF additive pipe to be locked <br />with padlock. <br />Francisco escorted me to the waste storage. Observed the one-third 55-gallon <br />drum of contaminated absorbent without a lid or label. Francisco reported he will <br />be transferring to a DOT-approved drum with label. The 55- gallon drum <br />containing the contaminated diesel had a lid but no label. Francisco stated that the <br />contaminated diesel will be placed in the 1000-gallon tank labeled as 'used oil'. <br />The diesel went through the drain which empties to an onsite well sump. Observed <br />the diesel in the clarifier mixed with water. Went to the west end of parcel, <br />observed drainage ditch to be dry. <br />Generated a complaint. <br />Issued Notice to Abate to Sundance Transportation and emailed to <br />sundancetrasportinc@gmail.com. <br />5105.rpt Page 2 of 5
