Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COWZIRJVO Site Location: 14749 N THORNTON RD <br />Activity Summary <br />Activity Date Recorded by <br />12/09/2020 Called Bryon Miller, (Sundance Transportation Dispatcher, RP Contact) MURO <br />916-870-8885 to ask if a licensed company has been contracted for the cleanup. I <br />let him know that there is an upcoming rain event, so this needs addressing. Bryon <br />told me their insurance company opens at 9:00 am, he will be contacting them as <br />well as Ben Khatirine to contract the facility's usual cleanup company, because he <br />doesn't know of any. <br />Called Ben Khatirine. He reported that Atlas was contracted for cleanup and they <br />are expected to arrive between 2 - 3PM. <br />Completed Prop 65 and emergency Response Record. <br />12/09/2020 Called City of Lodi Storm Water and Storm Drain at 209-368-5735. I was <br />transferred to Street and Storm Supervisor, Jesse Barnett. Jesse Barnett told me <br />that the facility address was outside their jurisdiction; .it was either county or state <br />due to it being close to hwy 5. <br />Called Justin Padilla (San Joaquin Storm Water Pollution Prevention) <br />209-468-2907. Left detailed message. <br />MURO <br />12/10/2020 Completed narrative and filed photos to CUPA inspection photos. MURO <br />12/10/2020 Called Ben Khatirine for update. Ben stated Atlas Environmental will be onsite later MURO <br />this afternoon to finish pumping out the clarifier and will return first thing Friday <br />morning to jet and clean the line. <br />Ben would like to know if it is okay that they perform cleanup of the pavement and <br />strom drain. <br />Discussed with Lead Senior Elena Manzo (EHD). <br />Called Ben with response. The facility is a permitted hazardous waste generator <br />with trained staff. They are able to do the clean up using a power washer. Things <br />to take into consideration, if the rinsate is not collected and allowed to drain into <br />the clarifier, it may overfill and empty into the drainage ditch. Then, facility will have <br />additional cleanup. Ben is aware and stated that the switch that empties the <br />clarified remains off. <br />Called Brian Millman (Senior Geologist at Advanced Geo) who is working with <br />facility with incident to request contact information for Atlas Environmental. <br />Brian provided the following contact: Bobby Farrington (Atlas Environmental) <br />209-355-5645. <br />Began photo log. <br />5105.rpt <br /> Page 3 of 5
