Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COWZIRJVO Site Location: 14749 N THORNTON RD <br />Activity Summary <br />Activity Date Recorded by <br />12/11/2020 Called and left a message for Ben Khatirine (Owner) at 916-261-6194 regarding BAKER <br />today's clean-up. <br />Called Bobby Farrington (Atlas) at 209-355-5645 and spoke regarding the <br />clean-up. Bobby Farrington said that yesterday they pumped out 1500 gallons of <br />liquid from the clarifier. He said the clarifier and lines are severely neglected so he <br />doesn't know how clean they can get them. His team started clean-up at 7:30 am <br />this morning and should be done around noon. Waste will remain on site and be <br />sampled by Advanced Geo. Waste will most likely be disposed of as non-RCRA <br />hazardous waste. Supervisor on site is Collin (209-628-3906). <br />Received call from Brian Millman (Sr. Geologist at Advanced Geo) at <br />209-482-8556. He said the clean-up is taking longer than expected and will notify <br />me when I should come out for verification. I let him know that I need to see inside <br />the drains and clarifier to verify they are clean. He said there was a lot of debris <br />inside and wanted to know if they need to remove that. I told him I would discuss <br />with my supervisor and call him back. <br />Discussed with Elena Manzo (Lead Sr. REHS). <br />Called Brian Millman back and let him know that any impacted debris needs to be <br />removed. If it is not impacted it does not need to be removed. <br />Received phone call from Brian Millman that clean-up is ready for verification. Left <br />office at 11:51 am and arrived on site at 12:12 pm. Met with Daniel Villanueva <br />(Senior Geologist with Advanced Geo). Trough and both lines to clarifier were <br />triple rinsed with Purple Soap & Degreaser. Sump and clarifier were also pumped <br />out and cleaned. Approximately two feet of debris was removed from the clarifier. <br />No sheen was observed in any of the drains, sump or clarifier. No smell of diesel <br />was observed near the drains, sump or clarifier. Six totes (Five 275 gallon totes <br />and one 250 gallon tote) of waste was generated from the clean-up of the drain <br />system. Waste will remain on site pending analytical. Power washing of fuel <br />island was not done at the time of arrival. Worked on narrative and photo log while <br />power washing of fuel island was being done. Verified fuel island was clean when <br />power washing was complete. Waste was put in a 275 gallon tote. Tote had <br />approximately 30 gallons inside of it. Clean-up is complete. Left site at 1:45 pm. <br />12/14/2020 Received update via email from Lydia Baker (EHD) stating cleanup was complete. MURO <br />Read narrative on Envision Connect. <br />12/15/2020 Finished photo log and gave a copy to Claudia Muro (EHS) for file. BAKER <br />12/15/2020 Received photo log of clean up from Lydia Baker. Worked on some dailies. MURO <br />12/17/2020 Sent an email to Bryan Miller (Sundance Transport Dispatcher) at MURO <br />sundancetransportinc@gmail.com requesting a copy of the analytical report and of <br />the final disposal record. <br />12/21/2020 Emailed Ben Khatirine letting him know that we will need copy of disposal records MURO <br />and analytical reports once obtained. <br />5105.rpt Page 4 of 5
