Fuel was deposited into the 91 tank on 8/29/22, functionality was <br />observed on 8/30/22. <br /> <br />REGULATION GUIDANCE: The continuous monitoring system shall be maintained to <br />detect the entry of the liquid or vapor phase of the hazardous substance stored in the <br />primary containment into the secondary containment, and to detect water intrusion into <br />the secondary containment. <br /> <br />CORRECTIVE ACTION: Ensure all operated UST systems have monitoring system <br />capable of detecting entry into secondary containments at all times going forward. <br /> <br />Response: The facility will ensure all operated UST systems have a monitoring system capable of <br />detecting entry into secondary containments at all times going forward <br /> <br />Item #315 HSC 25290.1(e) Failure to maintain interstitial space to detect leaks before release to <br />environment (post-Jul 2004). <br />OBSERVATION: The 91 tank annular brine sensor was not approved by the EHD prior to <br />receiving fuel. Fuel was deposited into the 91 tank on 8/29/22, without conductiong <br />required testing of the 91 tank annular brine sensor. <br />Functionality testing was observed on 8/30/22. <br /> <br />REGULATION GUIDANCE: The interstitial space of the UST shall be maintained, as <br />designed, under constant vacuum or pressure such that a breach in the primary or <br />secondary containment is detected before the hazardous substance is released into the <br />environment. <br /> <br />CORRECTIVE ACTION: Ensure all operated UST systems have approved smart or brine <br />sensors going forward. <br /> <br />This is a Class II violation. <br /> <br />Response: The facility will ensure all operated UST systems have approved smart or bring sensors <br />going forward <br /> <br />Item #316 Item #316 CCR 2636, 2643(c)(1), 2666(f) Failure to install a line leak detector. <br />OBSERVATION: The pressurized piping on the 91 tank was not equipped with an <br />automatic line leak detector approved by the EHD prior to fuel being placed in the 91 tank. <br />Fuel was deposited into the 91 tank on 8/29/22, without conducting required testing of the <br />automatic line leak detector. Functionality testing was observed on 8/30/22. <br /> <br />REGULATION GUIDANCE: Automatic line leak detectors are required to be equipped on <br />underground pressurized piping systems that carry a hazardous substance. <br /> <br />CORRECTIVE ACTION: Ensure all operated UST systems have approved automatic line <br />leak detectors going forward. <br /> <br />This is a Class I violation.
