An Emergency Procedures Plan Summary Poster has been mailed to your IW%OIPI on. This <br />poster must be completed and posted by an out -dialing telephone that Is located near each <br />hazardous waste collection area, and that would be used in an emergency. if you did not <br />receive your Emergency Procedures Plan Summary Poster, please call the Signing Hotline <br />at 1.866.222-9438 ext. 770-6699. Additional information about the Emergency Procedures <br />Plan Summary is provided in the f=ront Store Hazardous Waste Management training. <br />A Contingency Plan is required to be maintained at your store. This plan provides <br />additional direction regarding emergency response. The Contingency Plan and related <br />documentation must be In your EH&S Program Binder. The most up-to-date copies can be <br />found and printed from Store Resources. <br />In some cases, letters may be sent on your store's behalf to local emergency <br />response authorities, to familiarize them with hazardous waste generation <br />and storage at your store. If you receive copies of these letters, keep them <br />and any related documentation in your EH&S Program Binder. <br />CVS Pharmacy utilizes 3E Gompany to,work with the Store Manger as Emergency <br />Coordinators. 3E Company can be contacted for emergency response, as is listed an the <br />Emergency Procedures Pian Summary and/or your store's Contingency Plan at <br />1.888.464-2960. The phone number provided on the Emergency Procedures Pian <br />Summary can reach the 3E Emergency Response line, which is staffed by 3E Company 24 <br />hours a day, 7 days a week, 366 days a year. <br />Additionally, CVS Pharmacy has a CVS Health Safety Hotline that <br />is also manned by 3E Company and can also be reached at <br />1-888-464-2960. This hotline is designed to provide store <br />colleagues access to SDS information, poison and exposure control <br />information, or spill response assistance. u� <br />19 <br />
