3E Company's SDS service allows colleagues to receive live assistance 24-7-365 for all <br />SDS requests. <br />If an SDS is needed for spill response or poison /exposure information, the colleague should <br />contact the CVS Health Safety Hotline. Upon initial contact, the colleague will be asked if <br />he or she has a medical emergency or chemical spill. It is important for colleagues to <br />identify if either of these situations is applicable. If neither is applicable, the caller will be <br />asked for the following information to complete the SDS request process: <br />• Callers facility <br />• Name <br />• Phone number <br />Fax number <br />• Chemical/Product full label name and any applicable part numbers <br />Manufacturer <br />3E Company will deliver the requested document to the #ax number specified and provide <br />the caller with his or her request number. This request number can be utilized for any <br />follow-up activities related to this particular request. <br />When a chemical exposure has taken place, the colleague should contact the CVS Health <br />Safety Hotline for first aid advice. The colleague must advise that he or she has a medical <br />situation, if applicable. If poison control is requested, the colleague will be transferred to a <br />poison control specialist for first aid advice. <br />Note: NIPAA► protected information will remain confidential. <br />20 <br />
