The following is an itemized list of underground storage tank violations that have not been addressed for PACIFIC SERVICE STATION as of November10 , 2022 . Open violations from August 25 , 2022 inspection ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #205 - Failure to submit and have current UST Monitoring Plan readily accessible at the facility. OBSERVATION An approved copy of the monitoring plans were not available for inspection . REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times . CORRECTIVE ACTION : Immediately locate and retain a copy on site . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #208 - Failure to comply with the designated operator inspection requirements . OBSERVATION The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator inspection reports . The secondary containment testing was listed as 8/26/2020 . The correct date for the secondary containment testing is 8/26/2019 . REGULATION GUIDANCE: During the inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION : Ensure that designated operators performing inspections at this facility are including all of the required information on the reports . ----------------------------------------------------- OBSERVATION The designated operator failed to check and note all required items on the visual inspection reports . The incorrect and missing information included the following from the inspection reports : Owner/operator did not write their name in the report. -Mechanical float mechanisms were not selected . -The L3 (diesel submersible turbine pump sump) alarm on 12/22/2021 did not address the follow- up . Report was not signed by the owner/operator within 72 hours . REGULATION GUIDANCE: During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . Page 4 of 5
