"Hazardous Material Management" poster (The Poster) (IMG 3589-3590). When <br />asked how a waste determination is made for such items, Lucia Landeros, the store manager, stated she uses the <br />Sally Beauty Safety Data Sheets (SDS) application on the store's iPad, and if she still can't make a determination <br />using the SDS application, she will call the Sally Beauty Hotline phone number listed on The Poster. Lucia could not <br />locate an SDS for the wax, so she called the Sally Beauty Hotline number, but was transferred to a voicemail box to <br />leave a voice message. Lucia called a Cosmoprof store in Stockton, since she was unable to get an immediate <br />answer by calling the Hotline number. Lucia asked a store associate at the Stockton store which HW tote the Gigi <br />wax should be placed in. Lucia stated the store associate told her the wax goes into the State Toxic HW tote. The <br />store associate also emailed Lucia an SDS for Gigi Honee Wax Refill, which did not indicate the wax was <br />flammable, corrosive, or an oxidizer (IMG 3613-3615). Lucia placed the Gigi All Purpose Hard Wax into the State <br />Toxic HW tote at the time of inspection. <br />- Three containers of the Matrix Opti Curl Waving Activator, the Waving Formula, and the Neutralizer were found in <br />a bag inside the Corrosive Acids HW tote (IMG 3594-3595). The HW tote label did not include any of these types of <br />items as part of the contents of the tote. The Neutralizer contained hydrogen peroxide (IMG 3596). According to The <br />Poster, items containing hydrogen peroxide should be placed in the Oxidizers HW tote. Also, according to the Sally <br />Beauty Holdings, Inc. Guidelines for Accumulating Hazardous Waste poster and the Hazardous Waste Training <br />Guide, only one waste item/type should be placed in any single bag (IMG 3590, 3609). Lucia placed the Matrix Opti <br />Curl Waving Neutralizer into a separate bag and placed it into the Oxidizers HW tote. <br />- Three containers of the Iso Option 3 For Extra Firm Curl On Normal/ Resistant Hair Pre -Treatment, the Waving <br />Lotion, and the Neutralizer were found in a bag inside the Corrosive Acids HW tote (IMG 3602-3603, 3616). The <br />Pre -Treatment contained sodium hydroxide, the Waving Lotion contained ammonium hydroxide, and the Neutralizer <br />contained hydrogen peroxide (IMG 3617). According to The Poster, items containing sodium hydroxide or <br />ammonium hydroxide should be placed in the Corrosive Bases HW tote, and items containing hydrogen peroxide <br />should be placed in the Oxidizers HW tote. Also, according to the Sally Beauty Holdings, Inc. Guidelines for <br />Accumulating Hazardous Waste poster and the Hazardous Waste Training Guide, only one waste item/type should <br />be placed in any single bag (IMG 3590, 3609). Lucia placed the Iso Option 3 For Extra Firm Curl On Normal/ <br />Resistant Hair Pre -Treatment and Waving Lotion into a bag in the Corrosive Bases HW tote and placed the <br />Neutralizer into a separate bag and into the Oxidizers HW tote. The Corrosive Bases and the Oxidizer HW tote <br />labels did not include the types of items that were placed in the totes as part of the contents of the totes. <br />- A Goldwell Colorance 10P hair color aerosol can was found in a bag with a Matrix SoColor Cult Permanent Hair <br />Color inside the Corrosive Bases HW tote (IMG 3618-3621). According to The Poster, aerosol metal cans including <br />hair color should be placed in the Flammable Gases, Liquids, & Solids (Flammables) HW tote. Also, according to <br />the Sally Beauty Holdings, Inc. Guidelines for Accumulating Hazardous Waste poster and the Hazardous Waste <br />Training Guide, only one waste item/type should be placed in any single bag.
