The 2016 and 2017 records of <br />hazardous waste training for Lauren Lawson, the pharmacy manager, were not available at the time of inspection. <br />Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. <br />Ensure that employee training is properly documented. Submit the last three years of hazardous waste training <br />records for Lauren Lawson to the EHD. <br />This is a Class II violation. <br />605 <br />CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />The following was observed inside the CVS Pharmacy: <br />- A tote labeled Non -Viable Hazardous Pharmaceutical Waste, contained bagged loose pills, empty warfarin bottles, <br />and bottles of liquid medicines, but the tote label was not marked with the physical state, nor the contents (IMG <br />4010-4011). Lauren Lawson, the pharmacy manager, marked the physical state and contents on the label at the <br />time of inspection (IMG 4012). <br />- A tote labeled "Expired" was found containing two individually bagged loose pills, one of which was labeled as <br />metformin hcl (IMG 4013-4015). According to Lauren, these loose pills should have been placed in the hazardous <br />waste tote. <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />FA0023486 PR0541024 SCO01 01/30/2019 <br />EHD 22-03 Rev. 09/22/16 Page 7 of 8 RCRA Large Quantity Hazardous Waste Generator OIR <br />
