Please note that the weight of empty <br />warfarin/coumadin containers must be counted toward the total amount of hazardous waste generated and <br />clearly indicated on the manifests. <br />- An open blue bin labeled "Weekly Strong Pac Staging Bin," 1. Amber Vials," and "2. Loose Pills" was <br />observed on a counter in the pharmacy (IMG 4028). The blue bin contained two smaller open compartments, <br />one of which contained three loose pills individually bagged and labeled, and the other which contained at least <br />three loose pills individually bagged and unlabeled (IMG 4029). The blue bin also contained a bottle of Vimpat <br />(IMG 4030). According to Lauren Lawson, the pharmacy manager, pharmaceuticals placed in this blue bin are <br />processed once a week, at which point they are placed in the "Strong Pak" hazardous waste tote. Submit a <br />statement documenting the reason this container is labeled "Weekly Strong Pac Staging Bin." <br />An inspection checklist was provided to the facility operator on the day of inspection.
