OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 12/28/21, next inspection performed on 1/28/22 (31 days). REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. --------------------------------------- OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The designated operator incorrectly answered N/A when asked whether items listed in the previous month's report as needing follow up were actually corrected. -Floats and chains were not checked off as being present. -The designated operator stated they looked inside the sumps but the question regarding sump sensor locations was left blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a Class II violation. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 5/21/21 &5/10/22 contained an incorrect and expired ICC date for the DO that conducted training and the initial training date for the employees being trained was incorrect. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a Class II violation.
