WESTERN PUMP <br /> Petroleum & Lubrication Equipment Specialists <br /> Specific training on how to do their assigned job. <br /> Special training when they work with hazardous materials (see Hazard Communication) or <br /> certain types of machinery or other equipment. <br /> Training on how to use any personal protective equipment provided , such as respirators. <br /> Weekly Safety Meetings <br /> Upon completion of the meeting , make sure you and all attendees have signed the form . Forward <br /> the safety meetings to the company shop. <br /> Disciplinary Actions <br /> Violations of safety rules are considered unsatisfactory job performance and will be treated <br /> accordingly. Any employee who violates the policies and procedures as outlined in the Injury & <br /> Illness Prevention Program will be subject to the following disciplinary action . <br /> Supervisors must make sure our disciplinary policy is clear and uniformly enforced . This means <br /> that everyone is aware of our policy and everybody is treated equally. There are no exceptions to <br /> the rules . <br /> To document the disciplinary warning , use the Employee Disciplinary Warning (EDW) form in the <br /> appendix. All completed forms shall be forwarded to the main office for filing by SMART Safety <br /> Group. <br /> The employee' s supervisor and superintendent will review the EDW with the employee. The <br /> superintendent shall follow the four disciplinary procedures: <br /> FIRST OFFENSE: Written warning and retraining . <br /> SECOND OFFENSE : Written warning , training with possible <br /> suspension . THIRD OFFENSE: Disciplinary action up to and <br /> including discharge. FOURTH OFFENSE : Termination . <br /> Based on the severity of the violation , the verbal and/or written warning may be bypassed and the <br /> employee may be discharged for cause as provided in the current labor agreement. <br /> Each meeting of the Safety Business Meeting will review the EDWs from the current and prior <br /> period . <br /> Communication <br /> Training Programs <br /> It is our policy to provide training in safe work practices for all of our employees . No one should do a <br /> job unless he or she knows how to do it safely, and understands the hazards involved .
