OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The following documents were not found on site: -7/26/21 monitoring system certification test results -7/26/21 overfill prevention inspection results -2/15/22 overfill prevention inspection results -Work order for the 2/15/22 installation of overfill flappers -Work orders for the L6 (87 fill sump) 12/16/22 and L7(87 STP sump) 12/27/22 fuel alarms. According to the 1/11/22 designated operator report, both alarms were addressed by Elite IV pumping out water. No work orders were found on site for any work in the past 12 months. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The date for the overfill prevention testing is listed as 7/26/21. No records were found for any overfill testing on that date. Overfill testing was conducted on 2/15/22. -Owner/operator printed name was left blank on multiple reports. -The report states the designated operator inspected the sumps, but the question regarding sump sensor location was left blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed, document the dates they were done, and shall check all required items on the report. Within 48 hours of the completion of the designated UST operator visual inspection,the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
