REPORTING OBLIGATION <br /> All accidents must be reported to the Human Resources Department and to the employee's <br /> Manager immediately, but no later than the end of the work shift; prior to the individual leaving <br /> the facility. <br /> Manager/Supervisor must complete the Supervisor Accident Report form and the injured party <br /> should complete the Accident Report form. Both forms need to be emailed to <br /> assestprotection @aoodwil I-siv.ore <br /> The agency has the responsibility to conform to all federal,state,and local regulations regarding <br /> the control and disposal of hazardous waste/hazardous materials. <br /> Agency will be responsible for reporting annually to Unified Program Agencies with <br /> electronically reporting, collecting, and managing hazardous materials-related data as mandated <br /> by the California Health and Safety Code. If any change to Hazardous waste/material exceeds <br /> allowed limits,changes will need to be reported within 30 days of any change. <br /> TRAINING REQUIREMENTS <br /> Employee training is required for all employees and/or those handling hazardous materials <br /> and/or hazardous waste during normal and/or emergency operations. <br /> • Provide initially for new employees as soon as possible following the date of hire. New <br /> employees should not work in an unsupervised position that involves hazardous <br /> materials handling and/or hazardous waste management without the proper training. <br /> • Provided within six months of the date of hire for new employees for a large quantify <br /> generator <br /> • Ongoing and provided a least annually <br /> • Amended prior to a change in process or work assignments <br /> • Given upon modification to the Emergency Response/Contingency Plan <br /> 11 of 13 <br />
