The following is an itemized list of underground storage tank violations that have not been addressed for Harney Lane 76 as of September 08, 2022. Open violations from July 28, 2022 inspection 6/28/2022 designated operator inspection report and failed to check that they were responded to appropriately. The missing alarms include: -All alarms on 6/16/2022 . REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #210 - Failure to provide initial training , train every 12 months, and have a trained employee present, OBSERVATION The designated operator employee training record for the past 12 months was not found on site. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1 . Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection , monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training.
