The following is an itemized list of underground storage tank violations that have not <br /> been addressed for Harney Lane 76 as of September 08, 2022. <br /> Open violations from July 28, 2022 inspection <br /> 6/28/2022 designated operator inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: <br /> -All alarms on 6/16/2022 . <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #210 - Failure to provide initial training , train every 12 months, and have a trained employee <br /> present, <br /> OBSERVATION The designated operator employee training record for the past 12 months was not found on site. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1 . Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection , monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 6 of 7 <br />
