The following is an itemized list of underground storage tank violations that have not been addressed for fast and easy mart #103 as of September 08, 2022. Open violations from May 25, 2022 inspection Violation #107 - Failure to submit and maintain complete and current financial responsibility documents. 1 ) OBSERVATION Financial responsibility documents (certificate of financial responsibility document and a letter from chief financial officer) have not been submitted to the EHD. The most recent financial responsibility documents in CERS are dated 12/29/2020. REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers. calepa,ca. gov/, and upload the required documents. 2) OBSERVATION Current financial responsibility documents were not found on site. REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on site. CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a copy on site. ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #117 - Failed to submit "Secondary Containment Testing Report Form" within 30 days after the test. OBSERVATION Secondary containment testing was performed on 7/2/2021 and a test report was submitted on 9/8/2021 , one month late. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests.
