The following is an itemized list of underground storage tank violations that have not been addressed for fast and easy mart #103 as of September 08, 2022. Open violations from May 25, 2022 inspection Violation #209 - Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. OBSERVATION The Facility Employee Training Certificate dated 12/02/2021 did not include the date of employees assuming responsibility as facility employees and the designated operator ICC certification expiration date. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018, or within 30 days of hire if prior to October 1 , 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #214 - Failure to comply with UST operating permit conditions. OBSERVATION The facility is not in compliance with the written conditions of the operating permit. The overfill prevention equipment testing due by 10/2021 has not been conducted . REGULATION GUIDANCE: The owner and operator of underground storage tank (UST) systems shall comply with all requirements of the permit issued for the operation of the UST system. CORRECTIVE ACTION: Immediately take all necessary actions to bring these UST systems into compliance. Schedule for overfill prevention equipment testing now. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #319 = Failure to comply with overfill prevention equipment requirements. OBSERVATION Overfill prevention testing has not been conducted. Overfill prevention equipment testing was required to be conducted by 10/13/2018, it was conducted on 06/4/2021 (late) . The next overfill prevention equipment testing was required to be conducted by 10/2021 and it has not been conducted. REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is conducted within required time frames. Page 3 of 4
