A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: FMa <br /> TRACY 76 2420 W GRANT LINE RD,Tracy 16, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 2637.2(d&e), 2665(a&b) Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within <br /> 30 days. <br /> OBSERVATION: Overfill Prevention Equipment testing was performed on 9/27/2021 and a test report was <br /> submitted on 2/14/2022,4 months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 202 CCR 2712(i) Failure to retain a paper or electronic copy of the permit to operate at the facility. <br /> OBSERVATION:A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. <br /> This is a repeat violation, Class Il. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site.The following <br /> records were not observed on site: <br /> -5/22/2019 Monitoring System Certification results <br /> -5/20/2020 Monitoring System Certification results <br /> -3/18/2021 Monitoring System Certification results <br /> -5/11/2021 Secondary Containment results <br /> -9/27/2021 Overfill Prevention Equipment results <br /> -5/3/3021 work order for like-for-like replacement of the 87 leak detector by Elite IV <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
