Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO2FIRRWH Site Location: 550 W WRIGHT RD <br />Activity Summary <br />Activity Date <br />10/05/2020 <br />Recorded by <br />Received update from Lead Senior REHS Elena Manzo (EHD) who verified the MURO <br />cleanup on Saturday, October 3, 2020. Dave Larsen reported to Elena Manzo that <br />a manager or property owner had met with him (Dave) during the removal of the <br />oil-contaminated soil and was content with the cleanup. However, Dave did not get <br />contact information from the person. Elena asked that I contact the property owner <br />or manager to ask if they are content with the cleanup. <br />Called former property owner who was also listed as the person who manages the <br />property on current almond permit with the Ag Commissioner's Office (Ripudaman <br />Deniwal, 209-814-7518). I asked Ripudaman Deniwal if he has the phone number <br />to the current manager of the property. Ripudaman Deniwal said that the person <br />who met with PG&Ewas probably the current property owner. Ripudaman Deniwal <br />gave me the number to the current manager, Ed Pellegri, 209-609-0108. I called <br />Ed Pellegri and explained why I was calling. I asked if he was the person who met <br />with PG&E during the cleanup. He said he wasn't and state it was the owner, but <br />would not provide the property owner's, Dr. Vanowall, contact information. Ed <br />Pellegri ended call so that he may confrim with property owner. Ed Pellegri called <br />back within minutes stating that the property owner was content with the cleanup <br />done by PG&E. <br />11/19/2020 Created picture file for inspection on 10/3/2020, emailed the file to C. Muro MANZO <br />(assigned EHD inspector). <br />12/16/2020 Mapped Location on Envision Connect GIS Map. MURO <br />10/13/2021 Retrieved two photos from Dave Larsen on cell phone received on 9-28-2020 as a MURO <br />text showing the concrete slabs before cleanup and the spill area secured wiith <br />cones and yellow tape. Generated a photo log with those photos. <br />11/02/2021 Reviewed complaint for abatement. Edited narrative for typos and clarity. <br />Generated Emergency Reponse Record ( ER01) report and site map. Printed all <br />complaint documents. <br />Abating with 06 - for Cross - reference with Record ID PRO513829 (4040 West <br />Lane) for verification of disposal records. <br />Submitted to Elena Manzo for review. <br />12/27/2021 Called Alex Steele (PG&E) and explained that I will not be abating this complaint <br />to the 4040 West Lane facility since a 20-yard roll-off bin of contaminated soil was <br />generated. I let him know I will need a copy of a disposal record in order to abate <br />the complaint. I let him know I will be also send him an email with date and <br />location of the waste.
