Continue using dishwasher for sanitizing utensils until dispenser provides 200 ppm quat minimum. <br /> Repair/replace dispenser and email service report to dafonskaia@sjgov.org . <br /> Observed ice accumulation in walk-in freezer on ceiling,fan cover,walls, and shelves. Clean within 1 week and repair as <br /> necessary. <br /> Observed some leakage on counter under tea brewer. Per general manager the replacement part has not arrived yet and <br /> they have continued regularly cleaning leakage during operation.
