OBSERVATION: Leak detector testing was previously performed on 3/5/2020 and was completed on 6/21/2021, 3.5 months past due. REGULATION GUIDANCE: These tests are required once every 12 months. CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner. This is a Class II violation. Response: Correct date for the 2021 Monitoring System Certification is 3/2/2021, not 3/5/2020. The Line leak detector tester testing was performed on 6/21/2021 due to lack of calibrated line leak detector test equipment availability. Subsequent line leak detector testing will coincide with the annual Monitoring System Certification testing in March. 2. OBSERVATION: The 3/2/2021 monitoring system certification form submitted to the San Joaquin County Environmental Health Department (EHD) on 4/6/2021 incorrectly stated that line leak detectors were not installed in the underground storage tank system. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner and are correct. Submit a corrective action statement to the EHD. This is a repeat violation, Class II. Response: Unchecked check box in section 7 "Check this check box if line leak detectors ARE NOT installed." added note to section 9 "See August 21, 2021 leak detect testing" (see attached). 3. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. A gross leak test alarm occurred on 6/21/2021. The facility did not have any records showing the alarm was responded to and how it was addressed. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring
