The following items were written up in the inspection report that need to <br />be addressed: <br />1. OBSERVATION: Leak detector testing was previously performed on 3/5/2020 and was completed on 6/21/2021, 3.5 <br />months past due. REGULATION GUIDANCE: These tests are required once every 12 months. CORRECTIVE ACTION: Take <br />all necessary precautions to ensure testing is performed in a timely manner. This is a Class II violation. <br />2. OBSERVATION: The 3/2/2021 monitoring system certification form submitted to the San Joaquin County <br />Environmental Health Department (EHD) on 4/6/2021 incorrectly stated that line leak detectors were not installed in the <br />underground storage tank system. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days <br />of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br />performed in a timely manner and are correct. Submit a corrective action statement to the EHD. This is a repeat <br />violation, Class II. <br />3. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. A gross leak test <br />alarm occurred on 6/21/2021. The facility did not have any records showing the alarm was responded to and how it was <br />addressed. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring <br />records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br />readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance <br />and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. <br />4. OBSERVATION: The designated operator failed to document the alarm from the attached alarm history on the <br />6/21/2021 designated operator inspection report and failed to check that it was responded to appropriately. The <br />missing alarm was a gross leak test alarm (6/21/2021). REGULATION GUIDANCE: During the inspection, the designated <br />operator shall review the alarm history since the previous inspection, check that each alarm was documented and <br />responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms <br />
