Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 22, 2022 <br /> Facility Address: <br /> 35200 S CHRISMAN RD, Tracy <br /> Facility Name: <br /> SIERRA GOLF MANAGEMENT <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The site map did not have the emergency shut off locations identified <br />for the propane tank and the fuel tanks, all of the entrances and exits of were not identified on the site map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This violation was corrected on site. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The emergency response(ER) procedures were not complete. The emergency response and <br />contingency plan is missing the name and pone number of the nearest medical facility. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This violation has been corrected. <br />This is a repeat violation, Class II. <br />FA0014330 PR0520811 SC001 04/22/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 3 of 5 <br />1868 E.
