Eyes: Irrigate immediately. Recommend a pressurized eye/face wash unit. <br />Skin: Wash with soap and water <br />Breathing: Move affected to fresh air at once. If necessary, perform artificial <br />respiration and get medical attention immediately. When <br />available, use cylinder oxygen on the affected unless directed <br />otherwise by a Medical Doctor. <br />Swallowing: Identify the item swallowed. Follow first aid procedures or get <br />immediate medical attention. <br />5. Medical Treatment <br />Any site personnel requiring medical treatment but not requiring <br />ambulance transportation will be sent or transported to the nearest facility for <br />treatment. <br />6. Accident Reporting <br />All near misses, injuries or illness will be reported to the Safety and <br />Health Manager within 16 working hours after the time of the accident. An <br />example of the PHILIP ENVIRONMENTAL Accident Investigation Report <br />and the Medical Request/Exam or Treatment form to be used in the event of an <br />accident or injury requiring treatment are attached in the Report Forms section <br />of the Work Plan binder. Site Supervisors are responsible for the prompt <br />reporting of information to maintain the OSHA 200 form record of reportable <br />injury and illness. All injuries will be reported on the appropriate PHILIP <br />ENVIRONMENTAL form(s) and sent in to the Seattle corporate office with <br />the daily reports. Copies of all reports will be kept on site by the Site <br />Supervisor and a copy will be forwarded to the Project Manager and the <br />Project Supervisor. OSHA 200 reports are required to be copied to Corporate <br />Safety and Health at the end of each year. The form must be posted where <br />highly visible to employees by February I st of each year and left up through <br />the entire month. <br />7. Emergency Response for Uncontrolled Incidents <br />Spills - contain spill area, absorb liquids and drum <br />Fires - evacuate area if uncontrolled. If the fire is small, put out with fire <br />extinguishers. All extinguishers will be U.L. listed, Class 2A - 40BC or better. <br />umm, <br />
