The following is an itemized list of underground storage tank violations that have not been addressed for AM PM HAMMER/15 FOOD #83113 as of August 11, 2022. Open violations from May 25, 2022 inspection Violation #109 - Failure to submit "Designated Underground Storage Tank Operator Identification Form" within 30 days. OBSERVATION: A new designated operator, Wesley Parkingson, began conducting inspections on November 30, 2021 and notification was not provided to the EHD within 30 days of the change. - Missing DO reports from January -October 2021 - Missing DO reports March 2020 - Missing DO reports from May -December 2020 REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #115 - Failure to submit "Monitoring System Certification Form" within 30 days of completion of the test. OBSERVATION: Annual monitoring system certification, leak detector testing, and spill container testing was performed on 5/26/2021 and a test report was submitted on 7/20/21, 2 months late. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests.
