The following is an itemized list of underground storage tank violations that have not been addressed for AM PM HAMMER/15 FOOD #83113 as of August 11, 2022. Open violations from May 25, 2022 inspection Violation #208 - Failure to comply with the designated operator inspection requirements. OBSERVATION: The following were not attached to the designated operator inspection reports: - Missing alarm history receipt for November 2021, December 2021, January 2022, February 2022, March 2022, and April 2022. REGULATION GUIDANCE: The designated operator visual inspection report shall include: -a copy of documentation demonstrating action taken in response to each compliance issue identified during the previous inspection -a copy of the alarm history since the previous inspection -a copy of documentation demonstrating action taken in response to each alarm since the previous inspection -a list of each alarm since the previous inspection for which there is no documentation of the alarm condition and action taken in response CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection reports. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #210 - Failure to provide initial training, train every 12 months, and have a trained employee present. OBSERVATION: The designated operator employee training for all employees was not performed. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training.
