The following is an itemized list of underground storage tank violations that have not <br />been addressed for AM PM HAMMER/15 FOOD #83113 as of August 11, 2022. <br />Open violations from May 25, 2022 inspection <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The following were not attached to the designated operator inspection reports: <br />- Missing alarm history receipt for November 2021, December 2021, January 2022, February 2022, March 2022, <br />and April 2022. <br />REGULATION GUIDANCE: The designated operator visual inspection report shall include: <br />-a copy of documentation demonstrating action taken in response to each compliance issue identified during the <br />previous inspection <br />-a copy of the alarm history since the previous inspection <br />-a copy of documentation demonstrating action taken in response to each alarm since the previous inspection <br />-a list of each alarm since the previous inspection for which there is no documentation of the alarm condition and <br />action taken in response <br />CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection <br />reports. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #210 - Failure to provide initial training, train every 12 months, and have a trained employee <br />present. <br />OBSERVATION: The designated operator employee training for all employees was not performed. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br />maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br />individual performs the duties of a facility employees. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br />evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br />one of the facility employees present during operating hours shall have current training. <br />CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br />site, and submit a copy of the training records to the EHD. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Page 4 of 4 <br />
