COMPLAINANT ORDERED A PERSONAL SAUSAGE <br /> PIZZA AND A SALAD THAT CONTAINED CROOTONS, KIDNEY BEANS, GARBANZO BEANS,AND DRESSING.AT <br /> AROUND 3:00 PM, COMPLAINANT BEGAN TO EXPERIENCE STOMACH PAINS, NAUSEA, DIARRHEA,AND A <br /> HEADACHE. COMPLAINANT IS STILL FEELING STOMACH PAINS AT THE TIME OF COMPLAINT. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Per Elaine Gouskos (Manager), she was unaware of the complaint and did not <br /> receive any similar complaints. No illnesses were reported by any employees. Temperatures of sausage, mozzarella cheese, <br /> ranch dressing, garbanzo/kidney beans,diced tomatoes and pizza warmer were all within safe temperature ranges. Hot and <br /> cold water available at facility. Sanitizer available at facility. Soap and paper towels available at facility. <br /> Discussed report with Elaine Gouskos(Manager).
