WELL DESTRUCTION <br />14 The French Camp Golf Course Water System Well #2 has been inactive since <br />January 2006. Well #2 meets the definition of an abandoned well and shall be <br />destroyed, as per the California Well Standards and the San Joaquin County Well <br />Standards, The French Camp Golf Course Water System shall destroy Well #2 under <br />permit and inspection by the EHD by March 31,2017. <br />CERTIFIED OPERATORS <br />15 The French Camp Golf Course Water System shall utilize only personnel who are <br />certified in accordance with Title 22, Section 7107, California Code of Regulations, as a <br />Grade 1 Certified Distribution Operator (CDO) to make decisions addressing the <br />following operational activities: make alterations, repairs, and additions to the water <br />lines; disinfect, flush, drain, clean existing water lines; pull, rehabilitate and disinfect the <br />water well; stand-by emergency responses, maintain or adjust the flow and pressure, <br />including fire flows: investigate water quality problems, determine and control the proper <br />chemical dosage rates for the wellhead disinfection and distribution system. <br />16 The French Camp Golf Course Water System shall provide the EHD with a copy of <br />the valid CDO license from the State Department of Health Services of the personnel <br />who will be making operational decisions concerning the French Camp Golf Course <br />Water System. The EHD has Sam Hedge on file as the CDO/ CTO. <br />TREATMENT SYSTEMS <br />17 The French Camp Golf Course Water System shall have the EHD approval for any <br />installation or alterations of a treatment system prior to the French Camp Golf Course <br />Water System serving water that is being treated by the altered water system. A <br />procedural violation will be issued to the French Camp Golf Course Water System, if <br />the system is serving water that is being treated by an installed or altered treatment <br />system that the EHD has not reviewed and approved for installation. <br />This permit supersedes all previous domestic water supply permits issued for this public water <br />system and shall remain in effect unless and until it is amended, revised, reissued, or declared <br />to be null and void by the San Joaquin County Environmental Health Department. This <br />permit is non -transferable. Should the French Camp Golf Course Water System undergo a <br />change of ownership, the new owner must apply for and receive a new domestic water supply <br />permit. <br />Any change in the source of water for the water system, any modification of the method of <br />treatment as described in the Permit Report, or any addition of distribution system storage <br />reservoirs shall not be made unless an application for such change is submitted to the San <br />Joaquin County Environmental Health Department. <br />-5- <br />
