French Camp Golf Course Water orstem <br /> Page 2 <br /> 3) Provide the Environmental Health Department(EHD)with a completed copy of the required UWA public <br /> notification and the methods you are using within ten (10) days of receipt of this letter. <br /> 4) Continue with Routine monthly bacteriological monitoring. Upon receiving notice of a Routine sample <br /> testing positive for bacteria, instruct your laboratory to collect four Repeat samples WITHIN 24 HOURS OF <br /> BEING NOTIFIED using enumeration test methods and to test for chlorine residuals on all Repeat. <br /> 5) Instruct the laboratory to collect five Routine samples in the month following notification of aop sitive <br /> Routine sample, using enumeration test methods and to test for chlorine residuals on all five samples,as <br /> per the Sample Siting Plan. <br /> 6) Instruct your laboratory to submit to the EHD all bacteriological laboratory reports and the required chlorine <br /> residuals for samples collected for the French Camp Golf Course Water System in a calendar month no <br /> later than the tenth day of the following month. <br /> 7) Submit to the EHD within 30 days the enclosed Positive Total Coliform Investigation form with <br /> information on the current status of physical works and operating procedures,which may have caused the <br /> elevated bacteriological findings, or any information on community illness suspected of being waterborne <br /> and corrective actions taken to correct the violation. This shall include, but not limited to: <br /> A. Operating procedures that are or could potentially be related to the increase in bacterial count; pump <br /> repair/replacement, break in water lines, etc.; <br /> B. Any interruptions in the treatment process; <br /> C. System pressure loss to less than 5 psi (which will occur during a loss of power); <br /> D. Vandalism and/or unauthorized access; <br /> E. Physical evidence indicating bacteriological contamination of facilities; <br /> F. Analytical results of any additional samples collected, including source samples; <br /> G. Community illness suspected of being waterborne; <br /> H. Records of the investigation and any action taken; <br /> I. The name of the Certified Operator who performed the investigation and disinfection process; <br /> J. Method of disinfection of the water system and well. <br /> Please be aware that you may be billed at a rate of$122 per hour for any further enforcement activity required to <br /> obtain the sample results, documents, or actions EHD has requested within this letter.
