Response <br /> RTC form was signed and submitted 6/02/2022—see attachments Fast Easy Mart 103 RTC & corrective action <br /> 060222 (page 3) and Fast Easy 103 Underground Storage Tank Employee Training Certificate. <br /> Violation #117 <br /> Failure to submit a copy of the secondary containment test results on the "Secondary Containment Testing <br /> Report Form" to the UPA within 30 days after the test. <br /> Corrective Action <br /> Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely <br /> manner. <br /> Response <br /> RTC form was signed and submitted 6/02/2022—see attachments Fast Easy Mart 103 RTC & corrective action <br /> 060222 (page 2) and Fast Easy 103 SB 989 Report 070221. Future SB 989 reports will be submitted to CUPA <br /> within 30 days following inspection. <br /> Violation #319 <br /> Failure to comply with one or more of the following overfill prevention equipment requirements: Alert the <br /> transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an audible <br /> and visual alarm; or <br /> 2) Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction <br /> occurs when the tank is filled to no more than 95 percent of capacity; and activate an audible alarm at least <br /> five minutes before the tank overfills; or <br /> 3) Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity; <br /> or <br /> 4) Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br /> exposed to product due to overfilling. <br /> 5) Install/retrofit overfill prevention equipment that does not use flow restrictors on vent piping to meet <br /> overfill prevention equipment requirements when the overfill prevention equipment is installed, repaired, or <br /> replaced on and after October 1, 2018. <br /> 6) Perform an inspection at least once every 36 months, at installation and every 36 months thereafter. <br /> 7) Inspected within 30 days after a repair to the overfill prevention equipment. <br /> 8) Inspected using an applicable manufacturer guidelines, industry codes, engineering standards, or a method <br /> approved by a professional engineer. <br /> 9) Inspected by a certified UST service technician. <br /> 10) Maintain records of overfill prevention equipment inspection for 36 months. <br /> Corrective Action <br /> Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is <br /> conducted within required time frames. <br /> Response <br /> RTC form was signed and submitted 6/02/2022—see attachments Fast Easy Mart 103 RTC & corrective action <br /> 060222 (page 4) and Fast Easy Mart Stockton Full OPEI Report 2021-10-19. Next inspection is due by <br /> 10/19/2024 and will be conducted prior to that date with OPEI Report forwarded to CUPA when received. <br /> 3 <br />
