Time In: 9:21 am <br /> Time Out: 10:05 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: SUSHI VILLA BUFFET LLC Date: 09/16/2022 <br /> Address: 7916 WEST LN, STOCKTON 95210 <br /> Owner/Operator: LI,ZHIZHEN Telephone: (209)955-1396 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00055225 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ATE AT RESTAURANT AND HAS VIDEOS OF COCKROACHES IN A REFRIGERATOR AT THE SIT <br /> DOWN BAR OF THE SUSHI BUFFET.THEY WERE AT EYE LEVEL IN THE SUSHI REFRIGERATOR. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Operator called for a re-inspection to re-open facility. <br /> Per operator pest control serviced the facility for 2 hours and the facility was deep cleaned for 6 hours. <br /> I observed sticky traps that are placed in compartments under buffet stations. <br /> I observed on juvenile cockroach on sushi station counter. <br /> Clean and sanitize area. <br /> Rice scoop in kitchen container was still present during inspection, operator discarded solution and scoops are to be washed <br /> and sanitized. <br /> Per operator,the pest control contract is for a year of service. <br /> Continue/increase pest control service as needed. <br /> Continue with items noted on the routine inspection. <br /> Re-inspection on the items noted on the routine inspection to occur on or after October 3, 2022. <br /> Ok to open facility. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Loui Su, <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0002696 C00055225 SC544 09/16/2022 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
