The incorrect and missing information included the following from the inspection reports: <br />-The printed name of the owner/operator was left blank <br />-The overfill date is incorrectly listed as 11/8/2021. Overfill was replaced and testing conducted on 8/16/22. <br />-Reports incorrectly mark the box indicating the site has floats mechanisms under the dispenser. <br />-The 4/22/22 report states N/A for alarms in the past month and does not document the 4/21/22 L2 (5 hp STP sump <br />sensor) fuel alarm. <br />-The 4/22/22 report indicates no sumps were inspected. The 4/21/22 alarm did not have a work order or any record <br />of service technician inspection, which would require inspection of the 5 hp STP sump. <br />-The 6/24/22 report documented the 4/21/22 alarm and looked inside the sumps, but did not mark the box stating <br />sump sensors were in the correct locations. <br />-The 6/24/22 report states no documentation was found on site regarding the 4/21/22 alarm, but did not include this <br />in Section 3 as a compliance issue requiring follow up. It was listed in Section 13 as a comment. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />OBSERVATION: The designated operator inspections for the following months were performed more than 30 days <br />after the previous inspection: <br />- Initial inspection on 11/29/21, next inspection performed on 12/31/21 (32 days) <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br />frames. <br />This is a repeat violation, Class II. <br />FA0003826 PR0231820 SCO01 11/10/2022 <br />EHD 23-01 Rev. 9/20/2019 Page 6 of 8 Underground Storage Tank Program OIR <br />1868 E.
