Keith Sample was found on lists from 2022, <br />2021, and 2019, all with different information listed. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br />Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />This is a repeat violation, Class II. <br />305 <br />HSC 25290.1, 25290.2, 25291 Failed to maintain secondary containment (e.g. failure of secondary containment <br />testing). <br />OBSERVATION: The piping under the dispenser does not have secondary containment. A portion of the piping is <br />outside the under dispenser containment (UDC). <br />REGULATION GUIDANCE: All underground storage tanks shall be designed and constructed to provide secondary <br />levels of containment for all hazardous substances released from primary containment. <br />CORRECTIVE ACTION: Immediately contact a properly licensed, trained, and certified contractor to install <br />adequate secondary containment under permit and inspection of the EHD. <br />This is a Class II violation. <br />319 <br />CCR 2635, 2637.2, 2665, 2712 Failure to comply with overfill prevention equipment requirements. <br />OBSERVATION: Overfill prevention testing was conducted on 11/8/21, but was due in October 2021. <br />REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br />13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br />CORRECTIVE ACTION: Ensure all future testing is conducted within required time frames. The next overfill <br />prevention testing for this facility is due by August 2025. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />** The testing is incomplete. The leak detector for the high flow (5 hp) piping was not tested. The technician did <br />not have the necessary tools to tap into the piping. In order to avoid additional violations, this testing must be <br />completed by November 30, 2022. Provide a notice of testing to the EHD at least 48 hours in advance.
