Obtain Engineer Designed System —Commercial addition/repair application prior to installation of <br /> system. The fee is based on the current schedule at the time of payment. The system shall be <br /> installed and inspected in accordance with this approved plan. <br /> 2. As part of package treatment, EHD shall require flow rate and monthly grab sample of total <br /> nitrogen, Total Suspended Solids (TSS) and biochemical oxygen demand (BOD) from Effluent. <br /> The results are to be submitted to EHD on quarterly basis. EHD may reduce the monitoring <br /> frequency based on the monitoring result after one year. <br /> 3. Visual and audio alarm is required and verified by EHD inspector. <br /> 4. The proposed leachfield area field area shall be barricaded so it cannot be driven over, parked <br /> on, or used as a storage area. <br /> 5.
